The province of BC along with island health is advocating for the use of a new form that complements an Advanced Directive. Called medical orders for scope of treatment (MOST), this one page document will become part of the electronic medical record at the hospital and with BC ambulance. It is meant as a guideline to assist with individual wishes. It is particularly useful for the frail elderly or those living with life threatening chronic illnesses, and may have specific instructions for care. The info you fill out in the MOST form should follow with your wishes for end of life care, as expressed in your advanced care directive and in discussion with your physician, family and friends.
